Default ultimate rare pull ratio?

I've never known and can't seem to find it anywhere. Exactly how hard are they to get?

Supposedly it's the same as Ultras

No... it's really 1 per box, and that estimate is REALLY accurate, at least ever since Tactical Evolution (or Force of the Breaker). I have gotten about 10 boxes ever since then and every one of them had only 1 ULT.

It's SLIGHTLY over 1 per box.

I've gotten a box with 2. 2 of the same card too. XD

The previous boxes (SOD-EN to STON-EN/FOTB-EN) had 2 Ultimates per box and 1 Ultra per box. But they switched that at FOTB-EN/TAEV-EN.

There's this other theory that ULT Rares are one in every 18 or 20 packs. But judging that every box I seen so far after the Ultra Rare up ratio, there's only 1 ULT Rare. And I've seen a LOT of boxes. There are 3 possibilities to your scenario:

The ULT pull ratio is greater than 1:24
UD messed up
The 2nd ULT Rare pack got transferred from another box

Dang.. if that's true then my luck from pulling an ULT teleport from a box with 7 packs left was REALLY good.

Also, I think the ratio for ULT rare pulls decreased because of the fact that ULT rares only exist for all the ultra rares in sets since Phantom Darkness so there wouldn't be a need for a high ULT pull ratio since there are only like 6 or 7 ultras per set now.

Before it was 2 ultimates and 1 ultra in a box but then they changed it to two ultras and one ultimate per box

Yes it used to be 2 ultimates and 1 ultra in a box. But that was when you could pull supers and rares as ultimates, and there was only about 7 ultras in a set. Now, it's 10 ultras in a set and you can only pull one ultimate which is an ultra.
