Evolution Stones

I have a question about evolution stones.
I have several, but I cannot figure out which stone evolves which pokemon.
Also, other than Eevee, can any other pokemon turn into several different pokemon, depending on the stone that you use?
I have a water stone and a leaf stone right now.
Which pokemon will either of those evolve?
And if you have a list of all the stones, and all the pokemon they evolve, that would be tremendously helpful.

I looked around for a list of which stones affect which pokemon but didn't find anything. Each generation has added species and evo features so it's a longer list now too. I've got to run now, but will try and put something together for you later---at least for the water and leaf stones.
